Getting started with your personalized nutrition plan is as easy as 1-2-3.
Step 1: Complete the New Client Questionnaire. Our office will contact you within 48 hours with information for completing the next steps below.
Step 2: Review the "Practice Polices & FAQs" and "Service Offerings with Prices" to decide which nutrition package is right for you and then schedule your appointment(s).
Step 3: Register for the Patient Portal (please look for your invitation from me) and complete the New Client Intake Packet. Note: This step may take a while so please allow enough time for you to thoroughly complete the forms and upload them to the portal 5 days before your appointment.
Health Insurance
Most insurance companies cover nutrition counseling and education when administered by a Licensed Dietitian Nutritionist (LDN). However, coverage is not guaranteed and we recommend you personally confirm your nutrition benefits for your individual plan.
Malaika is currently “in network” and contracted with the following health plans:
- Aetna
- Highmark BCBS
- UnitedHealthcare
- Medicare
For all other insurance plans, you may still be eligible for reimbursement if you have out-of-network benefits. We will provide invoices that can be submitted to your insurance company. Please note that coverage for nutrition counseling varies greatly by plan and may change from year to year. We recommend calling your insurance carrier prior to your visit to confirm your out-of-network nutrition counseling benefits. For “out of network” insurance, you will be responsible for payment at the time of service.
